I think it should perhaps be a level-based thing. If you've not gotten a warning yet, you're on level 1 of the disciplinary scale. The moment you get a warning, you progress one third of the way to level 2. Meaning you need 3 warnings to get to level 2, and then we shorten the stick. From level 2 you only need 2 warnings to get to level 3, and at level 3, you get tried before a court to determine your penalty.
That means there should be a court that everyone's happy with, however.
EDIT: But that also begs the question of: "Who gives the warnings?"
